FERPA and Student Privacy Policies
Notification of Students' Rights under FERPA
The Family Educational Rights and Privacy Act (FERPA) affords students certain rights with respect to their education records. These rights are:
1. The right to inspect and review the student's education records within forty-five days of the day the 91极品福利 receives a request for access. Students should submit to the registrar, dean, or head of the academic department (or appropriate official) written requests that identify the record(s) they wish to inspect. The 91极品福利 official will make arrangements for access and notify the student of the time and place where the records may be inspected. If the records are not maintained by the 91极品福利 official to whom the request was submitted, that official shall advise the student of the correct official to whom the request should be addressed.
2. The right to request the amendment of the student's education records that the student believes is inaccurate or misleading. Students may ask the 91极品福利 to amend a record that they believe is inaccurate or misleading. They should write the 91极品福利 official responsible for the record, clearly identify the part of the record they want changed and specify why it is inaccurate or misleading. If the 91极品福利 decides not to amend the record as requested by the student, the 91极品福利 will notify the student of the decision and advise the student of his or her right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.
3. The right to consent to disclosures of personally identifiable information contained in the student's education records, except to the extent that FERPA authorizes disclosure without consent. One exception that permits disclosure without consent is disclosure to school officials with legitimate educational interests. A school official is defined as a person employed by the 91极品福利 in an administrative, supervisory, academic, research, or support staff position (including law enforcement unit personnel and health staff); a person or company with whom the 91极品福利 has contracted (such as an attorney, auditor, or collection agent); a person serving on the Board of Trustees; or a student serving on an official committee, such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibility. Upon request, the 91极品福利 discloses education records without consent to officials of another school in which a student seeks or intends to enroll.
4. The right to file a complaint with the U.S. Department of Education concerning alleged failures by the 91极品福利 to comply with the requirements of FERPA. The name and address of the office that administers FERPA is:

Note: Under Federal Statute, the 91极品福利 is authorized to and may release records to other institutions without notification to the student, when the student is applying for admission.
Request to Prevent Publication of Directory Information
Students may inform the 91极品福利 in writing of the student's desire to prevent publication of such 鈥淒irectory Information鈥 or release of such information except as required by law. Appropriate forms for such action are made available by the Office of the 91极品福利 Registrar.
Caution: Until the 91极品福利 can develop the necessary sophistication in our data systems, a student's request to prevent the release of publication of some of the items of 鈥淒irectory Information鈥 may result in preventing the publication of all items on that list, including graduation lists, honors, and award lists. The student can help avoid such errors with a gentle reminder to the Office of the 91极品福利 Registrar.

Student Communications and Records鈥擯rivacy and Access
Students have the right to access their student records that are on file in the Office of the 91极品福利 Registrar. Students requesting access to their file information, or a third party requesting information held in a student's file, have the right to a response from the Office of the 91极品福利 Registrar within 30 days if the student provides written consent. Should the record include information about more than one student, only the information pertaining to the student making the request will be given.

The disclosure or publication of student information is governed by the policies of Florida State 91极品福利 and the State of Education within the framework of state and federal laws, including the Family Educational Rights and Privacy Act of 1974 (FERPA). Phone Monitoring
Students should be aware that phone calls placed to Florida State 91极品福利's interactive telephone network may be periodically monitored to ensure that the appropriate quality control is maintained.
Parental or Third-Party Access to Records
Students may give a designated parent(s)/guardian(s), or other third parties (i.e., sibling, spouse, etc.), authority to review their 91极品福利 financial status, grades, transcript, student profile. Granting access to a parent/guardian or third party to view information in this manner also authorizes 91极品福利 personnel to discuss those records with the designated individual(s). Students should be aware that any individuals granted such access may see the selected student's academic records, including but not limited to preferred name, gender identity, personal pronouns, classes, grades, billings, etc.

Student enrollment verification is granted only through the submission of an official request. Students who require enrollment verification should visit /records/certification for further instructions. Written requests may be submitted directly to:

Statement Concerning Disclosure of Student Information
The disclosure or publication of student information is governed by the policies of Florida State 91极品福利 and the State of Education within the framework of state and federal laws, including the Family Educational Rights and Privacy Act of 1974.
The written consent of the student is required for the disclosure or publication of any information that is: (1) personally identifiable of the student and (2) a part of the educational record. Certain exceptions to that generality, both in types of information that can be disclosed and in access to that information, are allowed within the regulations of the Family Educational Rights and Privacy Act, as described in the following paragraphs:
1. Subject to statutory conditions and limitations, prior consent of the student is not required for disclosure of information in the educational record to (or for):
A. Officials of the 91极品福利 with a legitimate educational interest. A school official is defined as a person employed by the 91极品福利 in an administrative, supervisory, academic, research, or support staff position (including law enforcement unit personnel and health staff); a person or company with whom the 91极品福利 has contracted (such as an attorney, auditor, or collection agent); a person serving on the Board of Trustees; or a student serving on an official committee, such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his/her professional responsibility;
B. Certain government agencies;
C. Accrediting organizations;
D. Certain financial aid matters;
E. Certain research circumstances;
F. Health and safety emergencies;
G. A court pursuant to order or subpoena, so long as the student is notified in advance of the 91极品福利's compliance; and
H. As otherwise provided by law.
2. Subject to statutory conditions and limitations, prior consent of the student is not required for disclosure of certain types of information for:
A. Portions of the educational record for which the student has signed a waiver;
B. Portions of the educational record that are exempted by law including records of law enforcement agencies of the 91极品福利; employment records of the student within the 91极品福利 as long as the student's employment is not predicated upon his or her status as a student; personal records of instructional, supervisory, or administrative personnel; and alumni records related to that student; and
C. Records transmitted to another school or school system in which the student seeks or intends to enroll, since the 91极品福利 generally forwards these on request.

3. Prior consent of the student is not required for disclosure of portions of the educational record defined by the institution as 鈥淒irectory Information,鈥 which may be released via official media of the 91极品福利:

- Date and place of birth
- Official FSU email address
- Local address
- Permanent address
- Classification
- Major
- Participation in official 91极品福利 activities and sports
- Weight and height of athletic team members
- Dates of attendance
- Degrees, honors and awards received
- Most recently attended educational institution
- Digitized FSUCard photo
Important: The information above, designated by the 91极品福利 as 鈥淒irectory Information,鈥 may be released or published by the 91极品福利 without prior written consent of the student unless exception is made in writing by the student.
Policy for the Use of Photographs and Videos in 91极品福利 Publications
Florida State 91极品福利 randomly and routinely photographs and makes videos on the main campus, branch campuses, and the international and departmental programs for educational and promotional purposes. These photographs and videos appear in official 91极品福利 publications and materials, which include but are not specifically limited to, General Bulletin (undergraduate and graduate), Registration Guide, Office of Admissions brochures, international program materials, departmental and college brochures, 91极品福利 Web sites, and other 91极品福利 information publications.
